Enter your information on this form then print the form. Registration includes: 1) a $50 check, 2) this completed registration form and, 3) initialed Health Department Sanitary instructions. These are all to be submitted simultaneously in order to count as a complete registration.

Specific Non-Profit organizations have sellable items that may not be duplicated by others: First Baptist Church of Jonestown
(sausage-on-a-stick), Rolling Hills Community Church (sno cones), St. Mary's Our Lady of the Lake Catholic Church (turkey legs),
St. Peter's Episcopal Church (BBQ), Travis Oaks Baptist Church (water), Lago Vista Lions Club (hamburgers and hot dogs),
Lago Vista Players (corn-on-the-cob and shrimp), Lago vista 4th of July Council (beer and T-shirts), Pop Warner Football and
Rotary North Shore Lake Travis (patriotic items).

Deadline for above listed non-profit organizations to register: June 1 in order to retain their protected items.

The 4th of July Council does not supply electrical cords. Only 110v volts are provided.

(4) Ice will be available for sale at the park. If you will need more than a minimal amount of ice, please contact
Ann Murrow and let her know in advance.

Decorate! Decorate! Booth decorations are highly encouraged! Judges will view your creativity and use of red, white, and blue.
Humor and originality are invited.
Location: Bar K Recreational Park. Set up: 7:00am; Sell: 11:30am until after the fireworks. Parking Permits, call 970-4400.
Park to the right of the Bark K Clubhouse.
If you are serving food at the event, all local health department requirements must be met. The Lago Vista 4th of
July Council assumes no responsibility for compliance.
Your signature below indicates your knowledge of the Local Health Department's Sanitary Instructions and Application.
If food is to be sold, a signed copy shall be returned to the 4th of July Council with your application fee and a signed copy
shall be prominently displayed on your booth.

Release from Liability and Indemnification
We wish to have a Booth. We, individual(s) or organization, Indemnify and Hold Harmless the Lago Vista Property Association,
the City of Lago Vista, and the Lago Vista 4th of July Council and its volunteers from any and all claims, suits, losses, damages, expenses on account
of the booths. Further, the noted individual(s) or organization will defend the Lago Vista 4th of July and the City of Lago Vista,
and its agents from all claims or bodily injury or property damage arising from their participation in the booths. We understand
that we are responsible for our own safety and actions. By checking the box below, and signing, we signify that we have read
this Agreement and understand and agree to all its terms.
